Provides free Boehringer Ingelheim medications for those who meet eligibility requirements.

Medications include

1. aggrenox,
2. aptivus,
3. atrovent,
4. catapres-tts,
5. combivent inhalation aerosol,
6. flomax,
7. micardis,
8. mirapex,
9. mobic,
10. spiriva handihaler,
11. twynsta, and viramune.

Anyone may call to initiate the application process.

Once the application is received both the patient and physician need to complete the form.

To apply the completed application form, prescription, and patient's income and/or citizenship documentation are required.

Complete financial re-application is required annually.

Renewal requests within the same year require only the application form and a prescription.

Upon Approval a 90-day supply is shipped to the physician's office.

All requests are reviewed and approved on a case-by-case basis.

Maximum of three-month supply may be provided per request.
