Overview:

To provide emergency services to individuals and families in times of need.

Applicants will need to provide the following:

1. Proof of Address,
2. Proof of Income,
3. Rent Receipt or Housenote,
4. All Monthly Bills,
5. All Medical Bills (Receipts),
6. Food Stamp Card (If you have one),
7. Birthdate and Social Security Numbers of All Persons In Your Household, and First Time Pharmacy
8. Must Bring Written Prescription or Bottle
